How To Use Us

HOW TO REGISTER

In order to use the surgery, you must first register. You must be living within the practice catchment area (Merton and Wandsworth). If you are unsure whether you are in our catchment area, please call reception.

If you have an NHS medical card, please bring it with you when you register. If you do not have a medical card, please talk to the receptionist and they will assist you. All patients registering with the practice will be asked to complete a medical information questionnaire. If you are registering for the first time (if you don’t have a medical card), please bring your ID and proof of address.

APPOINTMENTS

All appointments are made by phone or in person. We do not accept appointment requests via email. Appointments are normally made up to 2 weeks in advance. In some cases we will accommodate up to 4 weeks in advance.

Emergency appointments are made on the same day.

CANCELLING APPOINTMENTS

If you are unable to attend your appointment, we would ask that you contact Reception to cancel it. Please give at least 24 hours notice.

HOME VISITS

To request a home visit please telephone the surgery between 9:00 and 10:30 am. Visits are made where necessary by the doctors after the morning surgery.

SICK NOTES

A doctor’s sick note is only required if you have been ill and unable to work for more than seven calendar days. For shorter periods, your employer may issue you with a SC2 self-certification form or you may obtain this form from the surgery. The doctor must see you in order to issue a sick note.

SMOKING

Smoking is not permitted in any part of the building or grounds.
